Corporate Promotional Services & Fees for 2003

Professional services to local businesses to assist in raising health awareness of employees, at low cost, has been a focus of Northwest Wellness for five years. Guaranteed screenings allow for efficiency and correct staffing. Non-guaranteed fees are also available. Fees reflect staff, equipment, supplies, license fees and waste removal. Health education counseling and group reports are negotiable for each health-screening event. All test resutls are otherwise confidential and not supplied to the employer.


Policies:

  • Deposits are needed for large events in that supplies ordered for these events are nonreturnable, and have shelf lives. Deposits: are non-refundable once supplies are ordered, usually two weeks in advance. Deposits are 50% of guaranteed number of tests due 21 days in advance of the event.


  • Cancellations: A 10% service charge is invoiced to all cancelled guaranteed events without a 21 day notice. A $250.00 service charge is invoiced to non-guaranteed events without a 21 day notice.


  • Invoicing: Usually within one week.


  • Terms: 15 days net.


  • Parking Reimbursements for all parking fees, if any, related to the date of the event.


  • Travel Reimbursements may be required for event locations outside of the Portland Metropolitan area (Woodland, Oregon to Kelso, Washington & Hilsboro Oregon to Gresham, Oregon).


  • Multiple-Site Events (same company) when ordered together, receive a 5% discount. Additional discounts may be available for multiple services.
